The Take Me Home Program is a regional photo-based information system hosted by the Sheriff's Department and accessible by all Law Enforcement in San Diego. It is designed to assist Law Enforcement during contacts with the members of the community who have disabilities, such as but not limited to Autism, Dementia, Alzheimer's, Down Syndrome, Deafness, or any other Developmental Disabilities. It also provides directions for contacting the agency's telephone information specialists who can connect callers with such services as case management and adult protection and advocacy. Visit here.
